ContinuousCare

ContinuousCare is priced on a clear SaaS subscription model, with plans starting at approximately $7 per user per month and scaling based on practice size, features, and deployment requirements. For most users, there are no hidden setup fees; clinics and startups can launch with minimal overhead, and many plans include essential support, onboarding, and standard system maintenance at no extra charge. Optional services—such as advanced white-labeling (publishing apps in your own App Store accounts), custom domain mapping, premium API integrations, or dedicated regional cloud hosting for enterprise scenarios—may incur additional charges, which are transparently quoted as part of the custom onboarding process. Ongoing support, basic maintenance, platform updates, security monitoring, and backup are bundled into active subscription plans, while intensive setup or migration support, and specialized integrations, may add one-time or tier-based costs by project scope. Overall, ContinuousCare aims for cost transparency and scalability, ensuring clinics only pay for the tools and customizations they actually use.

ContinuousCare provides structured onboarding, self‑serve resources, and responsive expert support so new users can go live quickly and confidently. A dedicated help center offers step‑by‑step setup guides for clinic services, video and text consultations, remote monitoring, provider accounts, and branding/domains, making initial configuration straightforward for admins and staff. Teams receive guidance on configuring consent workflows, notifications, and care plans, with practical walkthroughs that cover scheduling, billing, and patient portal usage to ensure end‑to‑end readiness before go‑live.

For day‑to‑day assistance, users can access in‑app guidance and reach the support team directly for questions, demos, or troubleshooting, with contact channels published on the site and within the product. Patient‑facing apps also include support touchpoints to resolve access and onboarding issues quickly, reducing administrative overhead on clinic staff. Operationally, ContinuousCare emphasizes close coordination between product, technical, and operations teams so implementation issues are resolved rapidly and best practices are shared during and after rollout. This combination of guided setup, searchable documentation, and responsive human support helps clinics achieve fast time‑to‑value and smooth adoption across roles.

ContinuousCare protects data with layered security aligned to healthcare regulations, combining encryption, access controls, consent management, and regional hosting to minimize risk across the full care workflow. All PHI is encrypted with continuous security monitoring and regular backups, ensuring confidentiality and recoverability for clinical records, messages, and documents. The platform enforces HIPAA and GDPR compliance by design, embedding legally compliant onboarding with patient consent and privacy controls throughout portals and apps.

• Access is restricted via secure role-based permissions for providers, staff, and administrators, so each user sees only the minimum data needed for their role across scheduling, EHR, and billing modules. Patient-facing portals and mobile apps use authenticated access to protect health records, test reports, journals, and care plans, reducing exposure while maintaining usability for remote care and follow-ups. Operational safeguards include automated notifications and audit-friendly documentation flows that keep sensitive communications and updates within the secure system rather than ungoverned channels.

• To meet data sovereignty and performance requirements, ContinuousCare offers regional cloud hosting in the US, EU, India, and Singapore, with the option for dedicated cloud installations for enterprise customers requiring stricter isolation and custom controls. White-label deployments with custom domains maintain the same security posture, ensuring branded web and mobile experiences do not compromise encryption, compliance, or monitoring standards.

ContinuousCare releases platform updates on a regular and agile cadence, typically every few weeks, to ensure clinics receive the latest features, security enhancements, compliance updates, and performance improvements with minimal disruption. All updates are managed centrally by the ContinuousCare product team and rolled out automatically to users as part of the SaaS cloud-based service—no manual installation or downtime is required for clinics or staff. Users receive update notifications in the platform and can review release notes or support documentation to stay informed about new features, changes, or resolved issues. Critical security or compliance patches may be issued outside the regular cycle to ensure ongoing protection and standards alignment, maintaining a seamless, always-modern experience for healthcare teams and their patients.

ContinuousCare states that healthcare organizations retain ownership of all patient health information and clinical records entered into the platform, while the vendor (NeedStreet) acts solely as a data processor handling Customer Data under the customer’s instruction. Customer Data remains the exclusive property of the clinic or healthcare provider, and the platform’s analytics/operational telemetry is processed separately by the vendor for service operations and improvements.

For portability, the platform provides multiple export options during the active subscription and for a limited grace period after termination. Customers can self‑export individual patient summaries as PDFs, and request bulk exports in structured, machine‑readable formats such as CSV or Excel, with delivery via secure transfer mechanisms. Certain subscription tiers also offer API access to retrieve data programmatically, governed by the same security safeguards as other processing; once exported, safeguarding and storage of the data becomes the customer’s responsibility. After account closure, data remains available for 30 days to support migrations or compliance needs, after which active‑system data is deleted and backups are scheduled for deletion within up to 12 months unless law requires longer retention. Patients can request account/data deletion through the portal or app, which providers must process in line with applicable regulations and record‑retention obligations.

ContinuousCare offers flexible subscription terms that allow clinics and healthcare organizations to scale up or down as their operational needs change, with no disruption to platform access or patient care. Organizations can add or remove provider seats, upgrade to higher feature tiers, or transition to enterprise-grade solutions—such as dedicated cloud hosting or advanced white-labeling—at any point during their subscription. These adjustments can be made via the online account dashboard or by contacting support, and new tiers are billed on a pro-rata or next-cycle basis, so costs remain predictable and proportional to usage.

Reducing the scale of services, such as dropping provider seats or switching to lower-cost plans, is also permitted prior to the next billing cycle, with no penalties or long-term lock-in; customers retain access to their data and can export records as needed when downsizing. For organizations experiencing rapid growth or planning new digital health initiatives, ContinuousCare supports seamless migration to enterprise deployments, the addition of specialized modules, or multi-location support without needing platform replacements or major transitions. This ensures clinics maintain agility and control as their business evolves, all within HIPAA- and GDPR-compliant boundaries.

ContinuousCare subscription terms and conditions state that contracts operate on a renewal model, typically monthly or annual, with clear provisions for cancellation, upgrades, and data retention to protect the interests of clinics and healthcare businesses. Subscriptions auto-renew unless cancelled with the required notice, and customers can upgrade, downgrade, or cancel via the online account dashboard or by contacting support before the next billing cycle. Cancellation takes effect at the end of the prepaid term, with no additional charges incurred after the period’s conclusion.

If the contract is cancelled, users retain access to their data and platform features for the remainder of the paid term, after which accounts are closed and data export options remain available for a specified grace period—usually 30 days post-cancellation—before permanent deletion per privacy policy and legal obligations. Refunds for advanced payments are generally not issued unless platform service failures or breaches occur; exceptions require written request and review. Terms also specify the process for account deletion, export rights, and responsibilities for legal record retention after platform closure. Enterprise customers may have tailored negotiations in contract terms for long-term rollouts, dedicated cloud hosting, or advanced white-label scenarios, but all billed customers benefit from transparent subscription management, compliance safeguards, and service-level guarantees during active terms.

ContinuousCare is designed to meet stringent healthcare compliance standards, including full HIPAA compliance for privacy and security of protected health information in the United States and comprehensive GDPR compliance for data protection and user rights in the European Union. The platform enforces encrypted data storage, secure patient consent and onboarding, regional cloud hosting that aligns with data residency laws, and rigorous access controls—all supporting adherence to both U.S. and EU legal requirements. These compliance frameworks ensure patient data is properly secured, processed, and accessible only to authorized parties, and support customers in meeting clinical, operational, and regulatory obligations for digital health globally.
